Management, auditors, and anti-fraud professionals are regularly monitoring for the red flags of fraud in organizations more today than ever before. While that is helping organizations fight fraud, not everything that presents as a red flag ultimately turns out to be fraud. Simply telling management or the client that it wasn’t fraud is not enough. You must be able to tell them what actually happened and caused the red flags. Red flags only tell us where there is an issue, not what is causing it or why and when it isn’t fraud the “what” and why” still need to be determined. That is where Root Cause Analysis comes in.
This course will introduce the various tools and methodologies used for root cause analysis and show how to apply them to understand the “what” and “why” behind issues. Participants will learn interactively through a combination of lecture, case studies, and exercises.

In this block, you will be introduced to Root Cause Analysis (RCA). The session will still start with a brief discussion about RCA including its history and background. You will learn the purpose of RCA and how it can be applied to fraud investigations as we explore the RCA process. You will also become aware of common challenges and mistakes associated with RCA application.
In this block, we will discuss RCA methodologies and techniques that can be utilized for Root Cause Analysis. You will discuss and practice applying SIPOC, fishbone diagrams, and mind mapping as tools that can assist in Root Cause Analysis.
In this final session, you will continue to explore more RCA methodologies and techniques that can be used for Root Cause Analysis. This session will include applying the RACI matrix and spaghetti diagrams. In addition to these tools, you will also spend some time discussing how to communicate results of Root Cause Analysis using the MoSCoW Method.
